APPLICATION FOR
ACCESS CODE NUMBERS
FOR USE OF COPIERS LOCATED AT MOST
CALIFORNIA WORKERS' COMPENSATION APPEALS BOARDS.
Thank you for applying for
your personal Access Code Number (ACN).
An ACN is
required to operate the copiers that have been
installed at most of the California WCAB District Offices. Your
ACN will allow you to use copiers at all the district
offices you select on the application. We do not
provide services at the following district offices:
Eureka, Fresno, Redding, Sacramento, Santa Rosa, and
Stockton.
Billing:
You
will be billed quarterly at a rate of $.20 per page.
Minimum Usage:
In
order to maintain your ACN, you must make a minimum of
50 copies per billing period (90 days) or agree to a
minimum charge of $10.00 per billing period. Document
Reproduction Services (DRS) will use this charge
to offset any accounting costs incurred. Please
select only the locations you frequent on a regular
basis. If you will not meet the minimum copy usage, you
may at your option use the coin-operated copiers that have
been furnished by Document Reproduction Services.
Application Deposit:
In order to
process your application, you will be required to pay a
$100.00 refundable ACN issuance fee, as well as a $10.00
activation fee for each district office you select.
This amount must accompany each application you
complete.
A
separate Application is required for each ACN you would
like issued. At your discretion, more than one
individual may share an ACN. If you obtain more than one
ACN, you may request a summary billing.
Within 10 days, you will
receive confirmation of your ACN along with simple
instructions for proper use. Please be sure that all
employees who will have access to the machines are aware
of the ACN and the proper procedures for their use.
